Regulatory History Form

When transfering to/from another jursidiction, or requesting a temporary/courtesy license for Nova Scotia or another jurisdiction you will require a Regulatory History form (sometimes referred to as a letter of good standing) be completed.  Each Canadian jurisdiction has their own forms, however, the information requested is consistent so that you may use either the of the sending or receiveing College's form.  

Download the NSCP Regulatory History Form here. Complete and sign the first section and email to info@nsphysio.com.  All Jurisdictions require completed forms be sent directly between Colleges.

Please allow 5 business days to process your request once the completed form is received.  You will be notified when the completed form has been sent.  If you have not received notification the form has been sent 5 business days after your completed form has been submitted please contact the office.
